First, you have rights as a patient to protect your health care information. The government has enacted protection under HIPPA. Our office policy for H.I.P.P.A. is available by clicking below. Please read it and if you have any questions or concerns, please bring them to our attention. By law, we are required to make this available to you.
Now that we have informed you about your rights to protect medical information, we can get started with the paper work. By clicking below you can download 4 pages that are necessary to be completed prior to treatment. Please fill them out as completely as you can prior to your visit. This information is needed to diagnose and treat you as effectively as possible. We realize this can be overwhelming but it is necessary. Please print these forms.
